Nominated support coordinator?

Is your nominated support coordinator listed on your plan anywhere? How do I find out who it is??

Hi Walter, thanks for your question. Firstly, you chose your support coordinator. Ideally what you need to do is be thinking about how you feel you can get the best from your plan. Coordination of supports helps to bring your plan 'to life' and connect you to the people and places in your community that will help you continue your good life.
Ask for support coordination in your planning meeting so your planner can have it included, as it does need to be funded. After you receive your plan you will meet with a Local Area Coordinator (LAC) who will help you to find a local service that will give you the kind of support coordination you are looking for.
